Participants under the age of 18 at the time of an on-campus event must have a completed form with a guardian's signature. More information will be provided closer to your event date.
Orientation Medical Information and Permission to Treat
All new students must provide evidence of vaccination against bacterial meningitis or must qualify for one of the two permissible exemptions prior to beginning classes at UNT. Immunization documentation should be uploaded to your my.unt.edu portal. Additional instructions can be found on the UNT Student Health and Wellness Center website.
Students who intend to register with the Office of Disability Access (ODA) under the
terms of ADA/504, or who request accommodations during their orientation session,
should check the appropriate boxes on their orientation registration materials. In

At Parent and Family Orientation, guests have the opportunity to purchase tuition draw tickets for an opportunity to win a free semester's worth of tuition at UNT. The drawing takes place during half-time of the Family Weekend football game in the fall semester. Tickets are only sold in-person at orientation and during Family Weekend. You do not have to be present at Family Weekend to win. For more information, visit the Family Weekend website.
The UNT Parent Association awards scholarships in the spring semester each academic year. Two different types of scholarships are awarded—the Scholastic Scholarship and the Draw, or External Scholarship. The number of scholarships given in each category depends on the availability of funds with a minimum of five $500 scholarships awarded in the Draw category. UNT departments can request to borrow a golf cart for daily use for program or event support. Individuals requesting a golf cart must be approved drivers through UNT Risk Management and have completed the OTP golf cart training. UNT students are only allowed to reserve golf carts if they are with a UNT department or UNT organization.
Following the completion of the approved driver process, departments may request to borrow a golf cart for daily use. Complete the following request form at least 1 week in advance.
