If your insurance plan is not listed or SHWC is not in-network with your provider, you can request an itemized receipt at check-out to submit for reimbursement based on your planβs out-of-network benefits.
We ask UNT students with commercial insurance plans to enter their insurance information into our patient portal at myOSH.unt.edu. Please follow these directions to upload your information. If you need assistance with uploading your information to the patient portal or have any insurance questions, please contact (940) 369-5446 or email SHWCInsurance@unt.edu.
All visits with a provider will have an office visit charge or copay. Many insurance plans will cover an annual physical or gynecological exam (usually referred to as a well visit), routine labs, and x-rays. Patients should refer to their insurance plan benefits for specific coverage and patient costs for these and other services. If you are unsure of your insurance coverage or need additional help, please contact the SHWC Insurance Department at (940) 369-5446.
Some insurance plans may require lab testing to be sent out to a specific reference lab. We will provide your insurance information to the reference lab, and they will bill your insurance. You may receive a separate bill from the Lab.
Most medical insurance plans do not cover specific services like dietitian visits, massage therapy, and travel medicine consultations. Additionally, discounted in-house programs, including monthly lab specials and walk-in STI testing are not covered by insurance; therefore, patients must pay at the time of service.
