The SHWC provides appointments for students traveling abroad for a UNT trip or personal travel. When making your appointment, tell the scheduler where you are going on your trip. You may be required to make a 2nd appointment.

What to bring to your appointment:

  • Immunization records
  • UNT travel forms
  • Student ID card

Immunizations must be given at least 14 days prior to traveling to be effective.

List of Immunizations offered at the SHWC:

  • Hepatitis A
  • Hepatitis B
  • MMR( Mumps, Measles and Rubella)
  • Tetanus
  • Adacel (Tdap)
  • Meningitis

Please note, there is a charge for medical providers to complete any necessary travel documents.
