Steps to Acccess Your Medical Records
  1. Go to the secure Patient Portal.
  2. Log in using your UNT EUID and password.
  3. Click on the Forms section.
  4. Select the Release of Information (ROI) under Medical Records.
  5. Complete the ROI form with all required information.
  6. Once processed, your records will be sent to your Patient Portal under the Messages section.

  • Medical records are retained for 10 years after the last activity.
